 Before designing a plan of care, it is important to retrieve a complete health history and health assessment. One technique that I would use to retrieve a pertinent health history is adaptive questioning. Adaptive questioning would allow me to develop rapport with my patient and gather information by asking general to more specific questions (Ball et al., 2022). The gradual shift in questioning allows the patient to reach a comfort level where he feels safe to communicate personal information without interruption of flow (Walden University, n.d.). Another component of this interview technique is echoing, which allows the provider to reflect and clarify the information by restating and turning the presented information into questions to elicit more information (Jones & Bartlett, n.d.).

           In addition, other communication techniques that can be useful for communicating to this patient are active listening and empathizing. Active listening is a non-verbal communication technique that demonstrates interest in what the patient is saying without judgment or bias (Slade & Sergent, 2022). Examples include removing distractions, making eye contact, nodding head, and echoing back to the patient key points of what you are hearing and/or taking notes. Moreover, empathizing is a technique that displays an emotional form of communicating through understanding and making the patient feel relatable like he/she isn’t the only person who is going through a situation (Slade & Sergent, 2022). I would use these techniques because it affirms to the patient that I recognize the present issue and validates their feelings about the issue.

            One risk assessment tool I would use to assess the patient’s pain is the Adolescent Pediatric Pain Tool (APPT). This multidimensional pain evaluation tool assesses the location of the pain by pinpointing that location on a body diagram or by using a graphic word scale (Özalp Gerçeker et al., 2018).  In addition, studies have shown that this tool can be used across cultures in children who possess language, verbal, and/or cognitive impairments (Özalp Gerçeker et al., 2018). Moreover, this unique tool measures not only location but intensity, quality, and patterns of how pain changes over time (Özalp Gerçeker et al., 2018). This tool is helpful in this case because it allows the patient to give a thorough account of his pain and address any psychological or emotional issues associated with being in pain. Often times it is overlooked that pain can affect other processes and aspects of the body.

           Some targeted questions that I would ask this patient while conducting this assessment are: 1. Describe what you were doing when this injury occurred.  2. Have you ever experienced pain like this, and if you did, how did you cope with it? 3. How do you feel this injury has affected your day to day and participation in sports, and how have you handled these changes? 4. Knowing that pain is different for each individual, what is your pain goal, and how can we work together to achieve this? 5. Have you noticed or have anyone disclosed any changes in your behavior, attitude, mental and/or emotional state?

Communicating with the patient using a patient-centered strategy that includes asking open-ended questions is likely to be the most successful method.
