Choose one of the following two assignments to complete this week. Do not do both assignments. Identify your assignment choice in the title of your submission.
Option #1: Creating a Case File
Review the case below of Keven, a 34-year-old client who is non-binary and uses they/them pronouns. Create a case file that includes the following key components as relevant:
- Medical Evaluation
- Psychological Evaluation
- Social History
- Educational History
- Vocational Information
You may need to be creative and add additional client details and information as you create the various sections of the case file. Chapter 9 of the text provides several examples that can be used for formatting each section.
Once you have completed the case file, write a 1-page essay that includes additional questions or information you would like to collect from Keven during the next interview that could be helpful in service planning. Also share why having a case file is important in case management.
Case
Keven has been a visitor to the Salvation Army Homeless Shelter for a few years, often with months between visits. Recently, Keven has been coming in a few times a week and seems to be less engaged with others in the shelter. From previous visits and meetings with Keven you are aware they were discharged from the Army 10 years ago after serving four years and spending time stationed in Iraq. Keven acknowledged feeling uncomfortable in the Army due to being required to identify as male.
Keven’s father passed away a few years ago and his mother passed away when Keven was younger. There does not appear to be other family members living in the community, although Keven does have a sister in another state that they do not have a good relationship with. Keven reports working ‘here and there’ at jobs like mowing lawns and construction. They would like to find a permanent place to live and want help finding a job.

Option #2: John’s Service Plan
As a Case Manager at a local mental health facility, you are preparing to meet with your new client, John. During the Intake interview, he shared his recent challenges at work with another employee and that he was referred to the facility for anger management services. He also shared that he is recently separated from his wife. This has caused John to sleep on a friend’s couch for the past few weeks and they have been going out in the evenings often. John’s friend, his previous college roommate, says John is welcome to stay with him for a few weeks. John is looking for a new place but has not been able to find any place that he can afford. Reviewing the Intake Summary, you note to ask additional questions about his living situation, the recent challenges at work, and going out with his friend. His file also includes testing that was completed by a psychologist when he was evaluated for ADHD last year.
After John’s second visit is complete, it is time to document the discussion. For your notes, you may use a bullet point or short essay answer format. If you use bullet points, make sure your notes are in complete sentences, detailed, and thorough. Include responses to the following:
- Initial assessment of John based on the information provided in the scenario.
- Case notes that include a summary of your second meeting with John and his responses to your prepared questions.
- A preliminary plan for John, including short and long-term goals following the SMART model.
- Services or resources in the community for John.
- Notes that outline topics for discussion at the third meeting.
