One of the key responsibilities of case managers is to coordinate services. What is involved in service coordination and why is it so important? What do you think the biggest challenges will be for you in service coordination? How will you address these challenges? How do tracking outcomes and monitoring client progress assist in the process of service coordination?
Your initial posting should be 250-500 words and must be submitted by Thursday, midnight (MT), of this week. By Sunday at midnight (MT), respond to two or more of your classmates’ initial posts in one of the following ways:
