Step 1 Read the case scenario.
You are working with an interdisciplinary team of health care professionals in preparation for the admission of a patient to your unit. She has a history of dementia and is known to wander and fall frequently. The team decides that it will be important to take a proactive approach to this aspect of the patient’s care and determine the best ways to address the issues of the patient wandering and her history of falls.
You all decide that it would be helpful to do some quick research on evidence-based clinical practice guidelines for care of the patient with dementia and also for falls prevention. It is important to keep all team members in the loop with the plan; however, not all members can be in the same place to conduct a briefing on the findings of this research. In addition, you think about ways that the team will stay informed of the care given by different members of the team and patient progress throughout her admission to the unit. You begin to discuss this with the other team members.
