- Based on one of the current National Patient Safety Goals, identify a practice violation or area for improvement that poses a threat to patient safety which has been observed within the student’s current area of practice or employing agency.
- Access The Joint Commission’s website https://www.jointcommission.org/standards/national-patient-safety-goals/.
- Select the applicable area of practice (behavioral health, home, hospital, nursing care center, etc.)
- Select Download chapter for the current National Patient Safety Goals of your selected practice area.
- Select one of the listed Goals and Element(s) of Performance to utilize as the quality goal for this practicum assignment.
- Interview a manager/director, quality department member, staff nurse, or other knowledgeable organizational member in the student’s current area of practice or employing agency regarding the topic of interest, current processes, quality benchmarks, and current quality efforts.
- Locate at least three (3) current evidence-based practice standards, clinical practice guidelines, or other applicable research to support the change in practice (see Resource List on subsequent page).
- Compare evidence-based standards to current practice within the student’s area of practice or employing agency. Identify areas for improvement in practice based on current evidence-based practice standards.
- Apply Lewin’s Change Theory to implement the proposed change in practice based on evidence-based standards and the current National Patient Safety Goals.
- Create a ten (10) to fifteen (15) minute multi-slide virtual presentation to the leadership team discussing the following:
- Introduction of self to leadership team
- Practice violation or identified area for improvement
- Summary of evidence-based practice standards
- Driving and restraining forces
- Activities for each phase of the change process (Unfreezing-Movement/Change-Refreezing)
- Resources needed for implementation including budget implications
- Strategies to reduce resistance and improve buy-in of stakeholders
- Plan for evaluation of change
- Describe how this initiative enhances a Culture of Safety within the practice area or employing agency.
