This is the clinical scenario
A 72-year-old obese male, a former smoker with PMHX of HTN, CAD s/p coronary stent placement six months ago, and HLD,
presented with complaints of shortness of breath over the past few months. Associated with fatigue, legs, and abdominal
swelling, coughing, occasionally wheezing, and orthopnea. Physical examination shows bilateral legs 2+ edema, distended
abdominal, hepatic congestion, positive JVD with HJR, displaced PMI 2 cm lateral, cap refill delayed, and hepatomegaly.
Diagnosis
Diagnosis: Congestive heart failure/heart failure (HF/CHF) brought on by fluid overload, uncontrolled hypertension, and
ischemic cardiomyopathy. The main focus is to remove excess fluids, decrease preload, and improve cardiac compliance
(Jaarsma et al., 2021).
