Option #1: The Case Management Process: New Client
Pat is a male client in his mid-30s who has been transferred to you from another Case Manager who has recently left the agency. Pat is already living in subsidized housing with his two children and has experienced a decrease in his weekly income due to his hours being reduced at his job as a result of staffing challenges.
Utilizing the Case Management Process, develop a plan for the first meeting with Pat next week and include responses to the following:
- How is the initial contact/meeting with a client different from a subsequent/ongoing meeting?
- What type of information may be helpful to gather at the initial meeting?
- What questions would you ask Pat to determine if there are additional services he may qualify for? How would you incorporate this information into a needs assessment?
- How would you include Pat in planning objectives?
- What is the role of the Case Manager during implementation?

Option #2: The Case Management Process: Ongoing Client
Katie has been an ongoing client at the homeless shelter and has received referrals for housing and food assistance in the past. Review her case below then develop a plan for meeting with Katie next week. Include responses to the following in your plan:
- How is this meeting, with an ongoing client, different from an initial meeting with a new client?
- What type of information may already be available to you since Katie is an ongoing client? Is there additional information you would like to gather from Katie at the next meeting?
- How would you include Katie in planning objectives?
- What is the role of the Case Manager when working with ongoing clients?
Case Example:
Katie is a 35-year-old woman who is currently homeless. She has experienced challenges interacting with others and does not have a regular job which has resulted in housing and financial challenges. Katie comes into the shelter once or twice a month for clothing or to visit the soup kitchen. She does not stay overnight in the shelter
Katie is an Iraq War veteran who served as a liaison to human resources and then as a paramedic. She received an honorable discharge in 2009 and lived with her parents for a few years. Katie left her parents’ home to live on her own and no longer has contact with her family.
She is often seen walking around the community and in the local grocery and most of her time is spent on the streets. Previously Katie worked as a janitor for several local businesses at night and slept in the places she cleaned. Katie has complained of hearing voices and denies using any medications or illegal substances. She worries that she is in danger from wild animals although she does not want to live in a house and be near other people. People scare her, noises scare her, and she has trouble sleeping.
